“No Shows” or “Late Cancellation Notice” Policy

If you do not show up for your scheduled appointment, and you have not notified us at least 24 hours in advance, you will be charged the full cost of the treatment as booked.

In the event that you are unable to keep your scheduled appointment with us, please contact us by phone at least 24 hours prior to your appointment. If you do not reach us, please leave a message on our voicemail. If we do not hear from you within this time frame, and you do not keep your appointment, the above policy will be applied. 

No show’s will be applied to the patient that has not showed up for their appointment within 15-20 minutes.

*We can understand that no one is perfect and there are circumstances that are out of your control (sudden illness, family emergencies, etc.); therefore, your therapist may make an exception to the above policies on those rare occasions*

 You can cancel your appointment by:

·      Calling our clinic at 604-370-0350 or leaving a voicemail;

·      Cancelling in person by coming into the clinic; and

·      Cancelling via the online booking link from our website: www.lifelogicintegrativehealth.com

 

 

 Late Arrival Policy

If you shall arrive to your appointment late, it will require that I end the session at the scheduled time. Meaning your session will unfortunately be shorter. I have reserved this time for you and have other patients scheduled after your appointment. Treatments will go no earlier or later than the scheduled appointment time allotted. Please be responsible for your own time.

 

 

Direct Billing Policy

When booking with us we will require your Extended Health Benefits information so we can Direct Bill to your insurance company. Please keep in mind that every insurance plan is different so you may or may not be required to do a co-payment or deductible. Most plans have a deductible amount that will need to be paid before the insurance company covers the cost of treatment (like a dentist visit). This deductible will reset every calendar year (January 1). The Payment Portal will give us the amount the insurance company will pay, you are responsible for any remaining amounts owing not paid by your insurance company.

For example, if you have coverage for 80% of your $120 visit fee, you will pay $24 (the portal will “pay” the remaining $96 to the clinic). Payment amounts depend on your individual coverage and portal response.  There may also be a deductible that you must pay before coverage begins. Check with your insurance company.

We do not check patients’ coverage, you are responsible for finding out this information. You can inquire on your coverage by reviewing your Extended Health Benefit booklet when you started your employment. You can also access this information if you are signed up for online access to your insurance company.

If the insurance portal is down or inaccessible on the day you have your appointment, we will bill you for the full amount and ask that you submit the bill to your insurance company.
